Reporting Concerns About Safety and Quality

Beech Brook is committed to quality of care and safety for its clients, families, visitors and staff.  We appreciate your feedback and are committed to excellent customer service. Please let us know if you have questions, suggestions or concerns. If you have concerns about client care or safety, we encourage you to let us know. You can contact:

If there are concerns that cannot be resolved through Beech Brook efforts, you may also contact Beech Brook’s accrediting body, the Joint Commission, directly with any questions or concerns you may have about safety or quality of care. Joint Commission assesses Beech Brook’s quality of care and safety. Patient safety and quality concerns can be reported as follows:
