Title:  Adoption/Foster Care Case Manager-

Requirements:  Recruit adoptive homes, supervise placements and complete foster to adopt and straight out adoptions for youth needing a permanent home. Ability to manager ODJFS, ODMH, Medicaid and probate court requirements for both foster care and adoption is crucial. Need to complete contract requirements for a variety of customers with varying priorities and procedures. May periodically provide foster care, therapy or family preservation services therefore the ability to multitask and the desire to learn new skills. Title:  Advanced Practice Nurse

Requirements:  Will work in collaboration with Director of Nursing and Medical Director provide assessment and medication management/education to residential and outpatient clients. Title:  Case Manager Therapist - Lorain Office

Requirements:  This is an on-call, PRN position which will include some evenings and weekends based out of our the Lorain County Office. Title:  Case Manager Therapist- School Based

Requirements:  Candidate will provide behavioral health therapy, mental health interventions and perform diagnostic assessments to at-risk youth with emotional, psychological, behavioral and/or trauma based issues within a school based environment. Title:  Youth Mentor - STRIDE Program

Requirements:  Candidate provides one-to-one mentoring in order to support the stability of youth's out-of-home placement. Mentor will provide youth with 60-120 hours of out-of-home activities over a 90-120 day period. Activities are geared to encourage improvemnent in youth's skills and coping abilities.
